Connecting community prevention and health care practice:
New resource released at press event
The Affordable Care Act is catalyzing major advances in the way we view health and our healthcare system. On the anniversary of the ACA, Prevention Institute is introducing a new resource to better align the efforts of medical institutions and communities to prevent illness, decrease injury and reduce inequity. On Thursday, Prevention Institute will release Community Centered Health Homes: Bridging the gap between health services and community prevention, offering concrete steps that health care leaders can take to actively support community change, improve health and deliver high-quality medical care.
  
Prevention Institute’s Executive Director Larry Cohen will release the report at a
forum and press conference Thursday. The event will feature U.S. Dept. of Health and Human Services Assistant Secretary for Health Dr. Howard Koh, HHS regional director Herbert Schultz, Jane Garcia, Chief Executive Officer at La Clínica, and Amy Compton-Phillips, MD, Associate Executive Director, Quality, Kaiser Permanente, alongside leading California-based foundations, clinicians and community groups. To mark the one-year anniversary of the Affordable Care Act, these leaders will highlight successes and groundbreaking efforts to build a system that promotes health for all people, not just healthcare for some.

There is growing awareness of the importance of community prevention in keeping people healthy in the first place. Community prevention is essential to improving population-wide health and safety outcomes for patients served by medical institutions and the community at large, reducing demand for health care services, improving efficiency, and reducing unnecessary costs within the medical system.

Community Centered Health Homes presents a set of research- informed recommendations for ways in which health care providers can engage in community prevention. The skills needed to engage in community change efforts are closely aligned with the problem solving skills providers currently employ to address individual health needs. Taking this comprehensive approach to health will reduce demand for resources and services; improve health, safety, and equity outcomes; and offer clinical providers the skills and strategies necessary to change the social circumstances that shape the health of their patients.
