Consider this scenario: Children begin showing up at a community clinic with symptoms such as respiratory infections, elevated lead levels, rashes, and bug bites. By asking questions and looking for patterns, staff at the clinic might find that these children all live in a nearby housing development with substandard conditions. Equipped with this information, the course of action could broaden to include partnerships with local housing rights activists and legal aid organizations. This revised approach not only treats the individual symptoms, but also prevents injuries and illnesses in the first place.
The webinar will provide an overview of the Community-Centered Health Home model, developed by Prevention Institute to highlight key steps clinical institutions can take to bridge the gap with community prevention efforts. Dr. Choi will share relevant experience from his clinical work in Oakland and his leadership of national physician organizations. We will discuss questions such as “What capacities does a health provider need to engage in community prevention efforts?” and “how can existing data be used to identify health trends?”
