Join Prevention Institute, the American Public Health Association, Public Health Institute, Trust for America’s Health, and our co-sponsors for a January 20th Web Forum on The Center for Medicare and Medicaid Innovation’s (CMMI) new Accountable Health Community Model (AHC) funding opportunity. The AHC model acknowledges the need to address underlying social determinants of health and equity and emphasizes clinical-community partnerships. The web forum will feature details about the AHC Funding Opportunity Announcement from CMMI staff as well as reflections and insights about emerging opportunities this new funding provides.
