The Blue Cross and Blue Shield of North Carolina (BCBSNC) Foundation is working to increase the capacity of the state’s safety-net healthcare organizations and communities to implement practices in line with the Community-Centered Health Homes (CCHH) model.
Prevention Institute conducted a landscape analysis, which was followed by information sessions and workshops hosted by the BCBSNC Foundation, to develop a commonly held understanding of community-based prevention, build a greater understanding of the CCHH model and related practices, and provide initial technical support. The Foundation then offered short-term grants to begin field-testing the CCHH model in communities, in order to advance a community-centered health framework in the state.
In this five-minute video, produced by Blue Cross Blue Shield of North Carolina Foundation, PI’s Larry Cohen describes the basic vision of the model, introducing it to health leaders in that state as they develop a demonstration project to put the model into practice.
