Communities of Care Year 6 Report
The Communities of Care (CoC) Year 6 Report highlights the impact of a six-year initiative led by Prevention Institute and funded by the Hogg Foundation for Mental Health to advance youth mental health and well-being across Greater Houston. Through 10 community-based collaboratives, the initiative empowered residents, youth, and cross-sector partners to address the root causes of mental health inequities by strengthening community leadership, fostering collaboration, and advancing upstream solutions.
Grounded in Prevention Institute's THRIVE framework, Communities of Care supported community-driven strategies that improved neighborhood conditions, expanded opportunities for learning, elevated youth and resident leadership, and advanced policy and advocacy efforts at the local and state levels. Despite challenges, including the COVID-19 pandemic, collaboratives built lasting partnerships, strengthened community capacity, and created sustainable models for collective action.
The report shares key achievements, lessons learned, evaluation findings, and recommendations for funders, practitioners, and community leaders seeking to build healthier, more resilient communities where children, youth, and families can thrive.
