The Community Sustainability Library

The Communities of Care Sustainability Library is a collection of practical tools and resources designed to help community collaboratives, nonprofit organizations, and resident leaders sustain their impact beyond grant funding. Developed by Wendie Veloz and in partnership with Prevention Institute as part of the Hogg Foundation for Mental Health's Communities of Care initiative, the library equips users with strategies to strengthen leadership, build partnerships, diversify funding, and advance long-term systems change.

Grounded in the Multi-Dimensional Sustainability Framework, the library features webinars, videos, planning templates, reflection tools, case studies, and real-world lessons learned from community collaboratives. Whether you are launching a new initiative, strengthening an existing coalition, or planning for the future, these resources provide actionable guidance to help organizations build resilient partnerships, center community voice, and create lasting change. While developed for Communities of Care collaboratives, the Sustainability Library is a valuable resource for any organization committed to advancing equity, community wellbeing, and sustainable collective impact.
