Published a new edition of the groundbreaking Community-Centered Health Homes paper

Prevention Institute published a new edition of Community-Centered Health Homes: Bridging healthcare services and community prevention. PI created the Community-Centered Health Homes (CCHH) model in 2011 to encourage and provide a structure for healthcare organizations to address the community factors that shape health, safety, and wellbeing.
